What are the available Header styles?

There are a few Headers to choose from when creating your account. Learn more about them here.

Doctors have different preferences when it comes to organising their consultation notes.

To accommodate these preferences, we offer several common header styles that can be selected.

This is a one time setup that you can do when creating your account.

Available Header Styles:

  1. SOAP (Popular)

    • Subjective: Patient's reported symptoms and concerns.

    • Objective: Examination findings and test results.

    • Assessment: Diagnosis or clinical impression.

    • Plan: Treatment and follow-up recommendations.

  2. HEIP

    • History: Patient history and reported symptoms.

    • Examination: Findings from physical exams and observations.

    • Impression: Clinical assessment or diagnosis.

    • Plan: Next steps, treatments, and follow-ups.

  3. HEIM

    • History: Details of the patient's symptoms and past conditions.

    • Examination: Physical findings during the consultation.

    • Investigations: Tests ordered or performed.

    • Management: Treatment plan and follow-up actions.

  4. HOPC

    • History of Presenting Complaint: Detailed account of the current issue.

    • Other Relevant History: Past medical history, medications, and lifestyle.

    • Physical Examination: Objective findings.

    • Clinical Impression: Diagnosis or working diagnosis.

Each of these styles is designed to streamline note-taking and align with different workflow preferences. Choosing the right format ensures clarity and efficiency in documentation.
